It is explained that Gilgamesh is a trans dimensional warrior.... Meaning that Good old Gilg, travels between dimensions searching for all master swordsmen.... Therefor... by that being said... the worlds (spira, gaia...ect) Are in different dimensions... as for the KH aspect of it well that is explained in the games that "bridges" are formed between the worlds and what not....
