There is no limit break glitch. You learn limit 2-2 by using limit 2-1, you learn limit 3-1 by killing enemies. Thus, if you kill enough enemies without using your limit break, you can easily jump to 3-1 without learning 2-2.
Can't say anything about the other glitches you mentioned, though...
Hell, with the right strategy you can beat him with a Level 5 Tifa solo with no materia, initial equipment and no accessory. Takes a lot of luck though:

One thing I want to point out about your Titanic example: the first time the Titanic was referred to as 'unsinkable' in the media was April 16th, 1912 -- the day after it sank. Certainly no one was talking about how God himself couldn't sink the boat before then. But even if people had, how...

- Fix the translation.
- Improve the character models in the field.
- Add a hard mode.
If there was a remake with those things alone, I would be a happy camper. Some of the glitches, like the MDef bug, make otherwise...
Yes. Neither of the WEAPONs are particularly difficult so long as you know what you're doing. With KotR is easiest, of course, but beating them without just takes strategy and time. Beat Ruby with a solo-Cloud equipped with the Ultima Weapon, Mystile and Ribbon. Started the battle with 4 Hero...
